## TL;DR

A rare case of shoulder deformity caused by a metastatic tumor in the rotator cuff from an oesophageal primary is reported for the first time.

## Contribution

This is the first documented case of an oesophageal cancer metastasis to the rotator cuff presenting as a unique shoulder deformity.

## Key findings

- A tumour deposit in the rotator cuff caused a progressive external rotation deformity of the shoulder.
- This case represents the first known instance of oesophageal cancer metastasizing to the rotator cuff.

## Abstract

We describe the first reported case of a tumour deposit within the rotator cuff presenting as a bizarre, progressive, and fixed external rotation deformity of the shoulder. It is also the first reported case to our knowledge of an oesophageal primary metastasising to the rotator cuff.
