Screening for red flags and modifying manual therapy techniques for pediatric patients
Virginia K. Henderson, Jenifer Dice, Doug Dendy, Jean-Michel Brismée

TL;DR
This paper reviews how to safely apply manual therapy to children, emphasizing red flag screening and the need for specialized guidelines.
Contribution
The paper introduces a modified red flag framework and a revised tool for pediatric manual therapy assessment.
Findings
Pediatric musculoskeletal systems are vulnerable to shear forces due to open growth plates and lower bone density.
Current safe force thresholds for pediatric manual therapy remain undefined.
The PUMT-2 tool combined with a modified red flag framework supports individualized pediatric care.
Abstract
This mini-review synthesizes evidence for safe and effective pediatric joint mobilization, discussing red flags, offering clinical recommendations, and addressing current research limitations. A PubMed and PEDro search, supplemented by foundational pediatric musculoskeletal resources, identified peer-reviewed articles and guidelines, which experts collaboratively evaluated. Pediatric musculoskeletal systems, with open growth plates and lower bone mineral density, are vulnerable to shear forces. Safe force thresholds remain undefined. Therefore, thorough history taking, systematic red flag screening, and adherence to International Federation of Manual and Musculoskeletal Physical Therapists guidelines are crucial. The refined Pediatric Utilization of Manual Therapy tool (PUMT-2, combined with the proposed modified three-tier red flag framework, supports individualized, developmentally…
