TL;DR
Tanzania improved maternal care by using virtual death reviews to identify and address causes of maternal deaths, leading to better accountability and quality improvements.
Contribution
Introduced and evaluated a national virtual maternal death review system to enhance quality of maternal care and accountability.
Findings
Virtual maternal death reviews (VMDR) were conducted for 687 maternal deaths, identifying obstetric haemorrhage as the leading cause.
Common modifiable factors included inadequate clinical competency and suboptimal provider practices.
VMDR prompted actions like redeploying specialists and improving essential supplies.
