## TL;DR

This paper introduces a technique to preserve the saphenous vein during lymph node dissection, reducing complications while ensuring cancer clearance.

## Contribution

A novel stepwise surgical technique for preserving the saphenous vein during inguinal lymph node dissection is proposed.

## Key findings

- Preserving the saphenous vein maintains venous outflow and skin drainage.
- Medial-to-lateral dissection allows vein preservation and oncologic clearance.
- The technique is easily reproducible and integrates into conservative surgical strategies.

## Abstract

Preserving the great saphenous vein during inguinal lymph node dissection maintains venous outflow and skin drainage while allowing complete oncologic clearance.

Saphenous vein preservation is a simple technical modification during inguinal lymphadenectomy.

Medial‐to‐lateral stepwise dissection allows vein and arch preservation while ensuring oncologic clearance.

Passing lymphatic tissue beneath the vein maintains venous drainage.

Technique is easily reproducible and integrates into conservative groin dissection strategies.
