This study demonstrates that analyzing two FIT samples increases detection rates of colorectal cancer and advanced adenomas compared to a single FIT sample.
Findings
33 cancers and 592 advanced adenomas would have been missed if only one FIT sample was analyzed.
PPV for cancer was 2.4% with one FIT and 2.7% with two FITs, while for advanced adenomas it was 19.8% vs. 26.2%.
Absolute FIT values were higher for patients with two positive FITs compared to one positive FIT.
Abstract
FIT is widely used for CRC screenings. Most Canadian programs only use one FIT sample when determining the need for a colonoscopy. It is unclear if an analysis of both samples in a standard two-sample kit improves detection of CRC and advanced adenomas. To determine the positive predictive value (PPV) of one versus two abnormal FIT in the detection of colorectal cancer and advanced adenomas. Over a five year period from 2020 to 2025, 84,489 completed FIT kits were received by the Newfoundland and Labrador colon cancer screening program. A participant completed two FIT samples and if one FIT was positive ( > =100ng/ml) a colonoscopy was arranged. Data collected included demographics, colonoscopy, pathology, and quantitative FIT results.
