TL;DR
Proton therapy for breast cancer is not cost-effective for most patients in Belgium, especially when compared to deep inspiration breath-hold photon therapy.
Contribution
This study provides a novel economic evaluation of proton therapy for breast cancer using a large-scale microsimulation model in a Belgian healthcare context.
Findings
Proton therapy was not cost-effective in any scenario under base case assumptions.
Under optimistic cost assumptions, proton therapy was cost-effective for only 3.6% of patients compared to free breathing photon therapy.
Proton therapy was not cost-effective compared to deep inspiration breath-hold photon therapy in any subgroup.
Abstract
This economic evaluation compares the cost-effectiveness of proton vs photon therapy for breast cancer treatment across various clinical scenarios. Is proton therapy cost-effective compared with photon-based radiotherapy, including deep inspiration breath-hold (DIBH), among patients with breast cancer in Belgium? In this economic evaluation using a state-transition microsimulation analysis with 1 million patients, proton therapy was not cost-effective compared with photon therapy under base case assumptions. In the most favorable cost scenario, it was cost-effective for only 3.6% of patients when compared with free breathing, and was not cost effective for any patients when compared with DIBH. These findings suggest that under current cost conditions, proton therapy appears unlikely to be cost-effective for most patients with breast cancer, particularly when DIBH is an option.…
