Alley Minton, Bailey Guest, Galina Wang, Cassidy Prewitt, Christopher Schrank

TL;DR
This study evaluated the effectiveness and safety of outpatient antibiotic treatment and stewardship practices at a VA hospital, finding high completion rates and cost savings.
Contribution
The study provides empirical evidence on the efficacy and cost-effectiveness of an OPAT program and ASP interventions at a Veterans Affairs medical center.
Findings
13% of Veterans on OPAT were readmitted, with 28% due to antibiotic failure and 21% due to adverse effects.
The program avoided 3,337 hospital bed days and 134 IV line days annually, with potential cost savings of $611,669.
The 30-day post-OPAT mortality rate was 1%, and the OPAT completion rate was 97%.
Abstract
Outpatient parenteral antimicrobial therapy (OPAT) refers to the delivery of parenteral antimicrobial treatment in at least two doses on different days without hospitalization required. Implemented antimicrobial stewardship programs (ASP) have reported significant reductions in unnecessary antibiotic prescriptions and improved patient outcomes. At the Salisbury Veterans Affairs Health Care System (SVAHCS), the OPAT program enrolls approximately 150 Veterans annually to which 86% of OPAT requests originate outside of VA care. The purpose of this study was to evaluate the efficacy and safety of OPAT along with ASP practices at SVAHCS.Clinical Characteristics and Most Common OPAT AntimicrobialsOPAT Indications Clinical Characteristics and Most Common OPAT Antimicrobials OPAT Indications This quality improvement project was conducted as a retrospective cohort study on eligible Veterans…
