P-2108. Clinical Outcome differences in Kidney Transplant Patients Receiving Letermovir Versus Valganciclovir for CMV Prophylaxis
YoungYoon Ham, Ali Olyaei, Ismael Aguirre

TL;DR
A study compares letermovir and valganciclovir for CMV prevention in kidney transplant patients, finding letermovir to be safer with fewer blood-related side effects.
Contribution
This study provides real-world evidence supporting letermovir's safety and efficacy in kidney transplant patients for CMV prophylaxis.
Findings
Letermovir reduced leukopenia and neutropenia compared to valganciclovir in kidney transplant patients.
Breakthrough CMV viremia was lower in the letermovir group, though no CMV disease occurred in either group.
Findings align with prior trials showing letermovir's non-inferiority for CMV prevention with better safety.
Abstract
CMV is a common herpesvirus that remains latent after initial infection. Adult seroprevalence ranges from 45-100% globally and is higher in transplant recipients (∼62%). CMV threatens immunocompromised patients, especially solid organ transplant recipients. Prophylaxis with valganciclovir or ganciclovir is standard but causes hematologic toxicity. Letermovir, FDA-approved in 2023 for high-risk kidney recipients, showed similar efficacy with fewer side effects.
