TL;DR
This study examines antibiotic prescribing patterns for acute cystitis in Michigan outpatients, finding common use of certain antibiotics and frequent deviations from recommended treatment durations.
Contribution
The study provides new insights into outpatient antibiotic prescribing practices for cystitis in Michigan, highlighting deviations from guidelines and potential areas for antimicrobial stewardship improvement.
Findings
Nitrofurantoin, TMP-SMX, ciprofloxacin, and cephalexin accounted for 93% of antibiotic prescriptions for acute cystitis.
60% of prescriptions for these antibiotics exceeded guideline-recommended durations, with ciprofloxacin showing the highest rate of prolonged use.
Prescribing patterns were consistent across subgroups but showed geographical variation and a notable shift to cephalexin in emergency room settings.
Abstract
Acute uncomplicated cystitis is a common illness in the outpatient setting requiring antibiotic therapy. While there are multiple treatment options for cystitis, some are preferred for antimicrobial stewardship (AMS) purposes, and outpatient prescribing practices are mostly unknown. This study aimed to describe antibiotic prescribing for uncomplicated cystitis among outpatients in Michigan.Figure 1:Agent SelectionFigure 2:Frequency of prescriptions with durations longer than guideline recommendation by different subgroups in Michigan Agent Selection Frequency of prescriptions with durations longer than guideline recommendation by different subgroups in Michigan The Michigan Department of Health and Human Services received medical and pharmacy claims data from IQVIA from 2019 – 2021.
