TL;DR
A new electronic health record order set for viral hepatitis testing improves test appropriateness and reduces unnecessary tests, especially among historically marginalized populations.
Contribution
A scenario-based EHR order set with clinical decision support was implemented to improve viral hepatitis testing practices.
Findings
HAV IgM and HBV Core IgM orders decreased by 27.2% and 28.5%, respectively, post-intervention.
Inappropriate repeat HCV Ab testing dropped by 34.6%, and test orders aligned better with clinical guidelines.
Special cause variation in SPC charts confirmed a significant reduction in inappropriate testing.
Abstract
Viral hepatitis (VH) screening is often suboptimal, with historically marginalized populations facing significant disparities in both testing and treatment. Paradoxically, providers frequently order inappropriate hepatitis tests, particularly acute-phase markers like HAV IgM and HBV core IgM in clinical scenarios where they aren’t indicated. Our specific aim was to improve local VH testing appropriateness and reduce wasteful ordering by implementing a clinical scenario-based electronic health record (EHR) order set.Order Set FeaturesOverall Test Volume Change Order Set Features Overall Test Volume Change We created an electronic order set for VH testing organized by clinical scenario rather than by virus type, to better align with clinical decision-making and evidence-based guidelines.
