TL;DR
High red blood cell counts in lung fluid can lead to false-positive results in a test for a fungal infection called aspergillosis, especially in patients with blood cancers.
Contribution
This study identifies elevated red blood cell counts in bronchoalveolar lavage fluid as a novel factor linked to false-positive Aspergillus galactomannan test results.
Findings
31 out of 64 patients (48%) had false-positive Aspergillus GM test results.
Patients with RBC counts >10,000 cells/µL had an 83% false-positive rate, compared to 30% with lower RBC counts.
The adjusted relative risk for false positives was 2.7 times higher for RBC counts >10,000 vs. <3,000 cells/µL.
Abstract
The Aspergillus galactomannan (GM) assay in bronchoalveolar lavage (BAL) fluid is used to diagnose invasive pulmonary aspergillosis, but specificity is variable. Lack of specificity is particularly important for immunocompromised patients, including those with hematologic malignancies, where a false positive can lead to misdiagnosis, delays in treatment and unnecessary antifungal therapy. Based on our clinical observations, we hypothesized that elevated red blood cell (RBC) counts in BAL fluid are associated with false-positive GM results.Table 1.Definitions of True and False Positive Aspergillus Diagnostic Categories.Table 2.Patient Characteristics for those eligible for main analysis.
