TL;DR
A sticker reminder helped doctors switch patients from IV to oral fluoroquinolone antibiotics more often without harming outcomes.
Contribution
A cluster-randomized trial demonstrated the effectiveness of a sticker reminder in promoting IV-to-PO fluoroquinolone switching.
Findings
The sticker intervention increased IV-to-PO switching among eligible patients (42.17% vs. 58.82%).
No differences were found in clinical cure rate, mortality, or hospital stay duration between groups.
Similar proportions of patients in both groups met IV-to-PO switching criteria.
Abstract
One of easily-to-implement antimicrobial stewardship strategies is switching of intravenous antibiotic to oral antibiotic (IV-to-PO switching). Fluoroquinolone (FQ) is considered a good candidate for the IV-to-PO switching strategy because of its good oral bioavailability, safety, and tolerability. A cluster-randomized trial at 45 wards in Siriraj Hospital, Bangkok, evaluated a sticker reminder intervention to promote IV-to-PO fluoroquinolone (FQ) switching in hospitalized adults. Wards were randomized to standard care (the control group) or the sticker reminder intervention (the intervention group), where daily bedside evaluations triggered sticker placement on patient charts if switching criteria were met. Physician discretion determined the actual switch. From July 1, 2020 – December 31, 2024, we enrolled 83 patients into the intervention group and 85 patients into the control…
