TL;DR
This study developed an electronic health record tool to improve communication of antimicrobial side effects for patients transitioning to outpatient therapy, resulting in better documentation of critical information in discharge summaries.
Contribution
A novel EHR-integrated intervention was implemented to standardize and improve documentation of antimicrobial side effects in Outpatient Parenteral Antimicrobial Therapy (OPAT) discharge summaries.
Findings
Post-intervention After Visit Summaries (AVSs) were significantly more likely to include therapy end dates, ID office phone numbers, and specific antimicrobial side effects.
The intervention led to a statistically significant increase in documentation completeness for antimicrobial recommendations and side effects in OPAT discharge summaries.
No significant demographic differences were found between pre- and post-intervention patient groups.
Abstract
Patients discharged on antimicrobial therapy often receive insufficient information about side effects requiring medical attention. This project aimed to implement an electronic health record (EHR)-based intervention to improve provider documentation of antimicrobial side effects in the After Visit Summary (AVS) for patients enrolled in an Outpatient Parenteral Antimicrobial Therapy (OPAT) program.Figure 1Intervention Overview.(A) Infectious disease provider view of intervention in Epic.
