Abstract
Karius testing (KT) utilizes cell-free DNA metagenomic next-generation sequencing to detect infectious organisms in plasma samples. More data on the clinical impact of KT in large cohorts are needed to understand its utility and relevance in different clinical syndromes. We examined the utilization, patient clinical characteristics, and clinical impact of KT in a retrospective cohort of adults at a large tertiary care center. Retrospective review of all KT completed for adult patients at the Medical University of South Carolina from January 2022 to November 2024. Clinical impact (positive, negative, or neutral) was assessed using pre-specified criteria. Descriptive analyses of microorganisms identified, patient characteristics, and clinical impact were performed. 228 patients had a KT; 94 (41%) were immunocompromised (Table 1).
