Abstract
Outpatient Parenteral Antimicrobial Therapy (OPAT) allows patients to complete extended courses of IV antibiotics in an outpatient setting. We established a formal inpatient OPAT transition service led by an infectious disease (ID) physician and an ID pharmacist.
