P-1488. Attitudes and decision making about older adult vaccination – results from a literature review to inform the design of a qualitative research study
Gitte Lee Mortensen, Oliver Martyn, Mandira Kar

TL;DR
This paper reviews factors influencing older adults' vaccination decisions in Western countries, highlighting drivers and barriers to improve vaccine uptake.
Contribution
The study provides a literature-based framework for designing qualitative research on older adult vaccination attitudes post-COVID-19.
Findings
Key drivers of vaccination include awareness, perceived susceptibility, and healthcare provider recommendations.
Barriers include lack of awareness, vaccine hesitancy, and structural constraints.
The pandemic increased awareness for some vaccines but also caused vaccine fatigue and hesitancy.
Abstract
Vaccination in older adults is crucial due to age-related immunosenescence and higher morbidity and mortality from infectious diseases. Despite the availability of effective vaccines for diseases such as influenza, pneumococcal disease, shingles, and respiratory syncytial virus (RSV), uptake remains suboptimal. This literature study explored the attitudes and decision-making processes related to older adult vaccination in Western countries, particularly post-COVID-19. A systematic literature search in PubMed focused on eliciting peer-reviewed articles published between 2020-2024, using terms related to vaccine hesitancy, acceptance, attitudes, and behaviors among older adults in Western countries. 28 relevant studies were included, providing insights into general attitudes, specific vaccine uptake, and the impact of the COVID-19 pandemic on vaccination behaviors. The findings informed…
