338. Evaluating the Clinical Impact of Molecular Drug Susceptibility Testing in Patients with Drug-Resistant Tuberculosis in Los Angeles County
Kyla Sherwood, Shom Dasgupta-Tsinikas, David L Fuller

TL;DR
This study shows that molecular drug susceptibility testing for tuberculosis leads to faster treatment decisions compared to traditional methods.
Contribution
The study demonstrates that molecular testing, especially pyrosequencing, reduces time to treatment for drug-resistant tuberculosis patients.
Findings
mDST was associated with an 11-day reduction in time to lab-informed treatment compared to pDST.
Pyrosequencing (PSQ) specifically reduced treatment initiation time by 17 days.
Only 45% of DR-TB patients received mDST, highlighting a need for increased access.
Abstract
Prompt identification of drug-resistant tuberculosis (DR-TB) is crucial for the timely initiation of effective therapy, but phenotypic drug susceptibility testing (pDST) takes weeks to provide results. Advanced molecular drug susceptibility testing (mDST) allows for rapid detection of mutations conferring drug resistance. This retrospective, cross-sectional study evaluated the association of mDST with clinical outcomes in patients with DR-TB in Los Angeles County from 2018-2022.Table 1.Demographic and clinical characteristics of the study population*Other includes Non-Hispanic Black, Native Hawaiian, and Other Pacific Islander.Abbreviations: ESRD = End-stage Renal Disease; TB = Tuberculosis; DR-TB = Drug-resistant Tuberculosis; INH-R = Isoniazid-resistant; PZA-R = Pyrazinamide-resistant; RIF-R = Rifampin-resistant; MDR = Multidrug-resistant; XDR = Extensively drug-resistantTable…
