Ricardo A Vargas-Figueroa, Derick Rodríguez-Reyes, Hiram E Luigi Martinez, Amanda S Vazquez-Lloret, Rafael Señeriz Ortiz

TL;DR
This review summarizes non-surgical treatments for common shoulder conditions, emphasizing exercise and other conservative approaches as effective first-line options.
Contribution
The paper provides an updated narrative review of non-operative management strategies for various shoulder pathologies based on recent guidelines and studies.
Findings
Exercise-based rehabilitation is effective across shoulder conditions, with outcomes comparable to surgery for subacromial pain.
Corticosteroid injections and hydrodilatation provide short-term relief for adhesive capsulitis when combined with mobilization.
Platelet-rich plasma remains investigational for glenohumeral osteoarthritis despite being used in some treatment plans.
Abstract
Shoulder disorders, including rotator cuff-related pain, adhesive capsulitis, impingement, glenohumeral osteoarthritis (GHOA), acromioclavicular (AC) pathology, instability, and biceps/superior labrum anterior to posterior (SLAP) lesions, are prevalent causes of pain, disability, and healthcare utilization. Contemporary care emphasizes non-operative strategies as first-line management, with surgery reserved for refractory symptoms or specific structural indications. The researcher conducted a systematized narrative review of adult non-operative management for common shoulder pathologies. Targeted searches of Medical Literature Analysis and Retrieval System Online (MEDLINE) (via PubMed), the Cochrane Library, and guideline repositories (American Academy of Orthopaedic Surgeons (AAOS)/American Shoulder and Elbow Surgeons (ASES)/American Physical Therapy Association (APTA)/American College…
