Abstract
Ensuring timely, quality, and culturally responsive access to medical and social care is essential to equity in meeting the diverse needs of older adults. Access influences every aspect of care delivery, from early intervention to ongoing management of chronic conditions, and plays a key role in improving health and wellbeing. This symposium will highlight how programs at Rush University Medical Center are breaking down barriers to care through the 4M’s of an Age-Friendly Health System Framework and expanding opportunities for older adults, caregivers, and providers.
