Abstract
The Aging, Life Innovations, Goals & Needs(ALIGN) Program is an innovative intensive ambulatory model of care that manages older “high risk” patients at risk for acute health decline with a thoughtful curriculum designed to highlight approaches to geriatric medical and psychosocial care.
