Grace Wittenberg, Martha Etzell, Peter Serina, Nichole Stetten, Ann Reddy, Ellen McCreedy

TL;DR
Bluestone's program helps manage emergency department transfers for dementia patients by improving communication and reducing risks.
Contribution
A novel ED early response program using ADT notifications to improve transitions for vulnerable ALC residents.
Findings
Patients with dementia and those on hospice benefit most from the program.
Improved communication between Bluestone and ED providers was a key strength.
Lack of ED staff awareness and timing challenges were identified as weaknesses.
