Dimethylarginines in pediatric CKD: clinical utility of ADMA and SDMA as biomarkers
Hülya Nalçacıoğlu, Murat Cihan, Hülya Gözde Önal, Demet Tekcan Karalı

TL;DR
This study shows that ADMA and SDMA levels rise with worsening kidney function in children with CKD, suggesting they could help assess disease severity.
Contribution
The study evaluates ADMA and SDMA as potential biomarkers for pediatric CKD severity and renal function decline.
Findings
ADMA and SDMA levels were significantly higher in children with CKD compared to healthy controls.
ADMA levels increased progressively with CKD stage, especially in stage 4 patients.
Both ADMA and SDMA showed moderate negative correlations with estimated glomerular filtration rate (eGFR).
Abstract
Pediatric chronic kidney disease (CKD) often presents no symptoms in its early stages, making timely diagnosis challenging. Asymmetric dimethylarginine (ADMA) and symmetric dimethylarginine (SDMA) are methylated arginine derivatives that reduce nitric oxide availability and have been suggested as potential early indicators of kidney dysfunction. This study aimed to evaluate the usefulness of ADMA and SDMA in pediatric CKD and to assess their association with renal function and disease severity. This single-center, cross-sectional, observational study enrolled 100 children aged 1–18 with CKD stages 2–4 and 70 healthy, age- and sex-matched controls between January and September 2023. Serum ADMA and SDMA levels were measured using ultra-high-performance liquid chromatography-tandem mass spectrometry (UHPLC-ESI-MS/MS). The estimated glomerular filtration rate (eGFR) was calculated using…
