## TL;DR

This paper reports the first known case of Nocardia wallacei infection in Cyprus, highlighting its rarity and treatment challenges.

## Contribution

The novelty lies in documenting the first confirmed case of N. wallacei infection in Cyprus.

## Key findings

- N. wallacei infection was diagnosed in a 77-year-old man with pulmonary disease.
- This is the first reported case of N. wallacei in Cyprus.
- The infection can affect both immunocompromised and immunocompetent individuals.

## Abstract

To present the first known case of Nocardia (N.) wallacei infection in Cyprus.

This case report describes the first known N. wallacei infection in Cyprus, diagnosed in a 77-year-old man with underlying pulmonary disease, along with the diagnostic workup and treatment.

N. wallacei is a rare subtype of Nocardia spp. with about 14 cases reported in literature. It equally affects immunocompromised as well as immunocompetent patients, causing both pulmonary and extrapulmonary symptoms (CNS, skin). First-line treatment includes trimethoprim/sulfamethoxazole (TMP/SMX), but there are trends of global resistance.

N. wallacei has rarely been reported as a cause of infection and this is the first known case reported in Cyprus.
