Abstract
to describe the development process and content validity evidence of an instrument designed for use by Community Health Agents during home visits to monitor individuals with Diabetes Mellitus and Hypertension in Primary Health Care. this methodological study was based on the Double Diamond model and included a literature review, the administration of questionnaires to community health agents and nurses, and content validity assessment by expert nurses. The study was conducted between September 2022 and October 2023 across ten Primary Health Care Units in a municipality in southern Brazil. a total of 60 professionals participated in the study.
