Development and Validation of a Prediction Model for Respiratory Failure in Patients With Sepsis-Associated Acute Kidney Injury (SA-AKI) Within 48 Hours of Admission
Bin Wang, Fengxiang Zhang

TL;DR
This study developed a model to predict respiratory failure in sepsis-related kidney injury patients within 48 hours of admission, aiming to improve timely interventions.
Contribution
A novel nomogram prediction model using clinical and biochemical indicators for early respiratory failure risk in SA-AKI patients.
Findings
The model achieved AUC values of 0.818 (training) and 0.795 (validation), showing strong predictive ability.
The model outperformed SOFA and NEWS models in predicting respiratory failure (p < 0.05).
Key risk factors included D-dimer, lactate, pro-BNP, and pulmonary infection.
Abstract
To identify patients with sepsis-associated acute kidney injury (SA-AKI) at high risk of respiratory failure within 48 h of admission and enable timely intervention to improve patient prognosis. Data from SA-AKI patients admitted to Dongyang People's Hospital between June 2012 and October 2024 were collected, including gender, age, and blood biochemical indicators at admission. Patients were randomly divided into training and validation groups. Independent risk factors for respiratory failure were identified in the training group, and a nomogram prediction model was developed. The model's discriminative ability was assessed using the area under the receiver operating characteristic (ROC) curve (AUC), and its calibration was evaluated using the GiViTi calibration plot. Clinical effectiveness was examined using decision curve analysis (DCA). Cross-validation was performed to test the…
