Abstract
Primary Health Care (PHC) has been a central focus in health policy debates, nationally and internationally. In this context, efforts aim to achieve an effective integration between PHC and the broader health system to ensure equitable, accessible, and needs-based care aligned with the real needs of the population. To describe the characteristics of the families participating in the study and to conduct a theoretical-reflective analysis of the challenges faced by PHC, as well as the responses required from service providers and local health authorities. The approach emphasizes family and community health to better meet the needs of vulnerable families in the Colombian context. A cross-sectional descriptive study was conducted.
