“Overdiagnosed or Misdiagnosed” – A Classic Case of Pseudohallucinations and Maladaptive Daydreaming When a Diagnosis of Adult ADHD Was Missed
Saima Jehanzeb, Amna Javaid

TL;DR
This case study explores how ADHD in adults can be misdiagnosed as a psychotic disorder due to symptoms like pseudohallucinations and maladaptive daydreaming.
Contribution
The paper highlights the overlap between ADHD and pseudohallucinations, offering a new perspective on misdiagnosis in adult ADHD cases.
Findings
ADHD symptoms like zoning out and daydreaming can mimic pseudohallucinations and dissociation.
Methylphenidate treatment resolved the patient's symptoms, indicating ADHD as the root cause.
Internal voices were reinterpreted as non-psychotic self-dialogue linked to ADHD.
Abstract
Aims: Background: Attention deficit hyperactivity disorder (ADHD) can present differently in adults and often goes misdiagnosed due to overlapping symptoms. Maladaptive daydreaming may also be misdiagnosed, as it shares similarities with dissociative symptoms, depersonalization, derealization, and temporal lobe epilepsy. A 19-year-old patient with a childhood history of trauma and isolation presented with hallucinations that began at age eight and progressed over time. She reported hearing up to 50 voices, described…
