Matilda Burton, Nilamadhab Kar, Annabel Ariyathurai, Mohamed Yaasir Mohamudbucus, Vijay Perla

TL;DR
This study shows that updating electronic health records with accurate psychiatric and physical diagnoses improves patient care and record quality.
Contribution
The paper presents a quality improvement project focused on enhancing diagnostic documentation in mental health electronic records.
Findings
Before the intervention, only 30.7% of patients had psychiatric diagnoses in the designated ECR section.
After updating, the mean number of psychiatric and physical diagnoses increased significantly (p<0.001).
Post-intervention, more ICD codes with higher character counts were recorded, indicating improved diagnostic specificity.
