TL;DR
This study tested a virtual coaching program to help adolescents with chronic diseases transition to adult care, showing it is feasible and may improve readiness and quality of life.
Contribution
The study introduces a novel virtual transition coaching intervention for adolescents with chronic diseases and demonstrates its feasibility.
Findings
The recruitment rate was 58% in gastroenterology and 38% in rheumatology clinics.
Only 7% of participants dropped out, and most attended at least 6 of 8 coaching sessions.
Preliminary data suggest improvements in transition readiness and quality of life in the intervention group.
