Weiping Li, Suoshi Jing, Bo Zhao, Wei Jiang, Bin Zhang

TL;DR
A 68-year-old woman with metastatic UTUC after surgery was successfully treated with Disitamab vedotin (RC48) for two years, showing no recurrence.
Contribution
Demonstrates the feasibility of long-term RC48 monotherapy as a treatment option for metastatic UTUC after surgery when other therapies fail.
Findings
RC48 monotherapy controlled the disease for 13 cycles with no recurrence in two years of follow-up.
The patient tolerated RC48 well with normal thyroid function and no significant side effects.
RC48 is a viable treatment option for selected patients with metastatic UTUC after RNU.
Abstract
For patients with metastatic upper tract urothelial carcinoma (UTUC), the preferred first-line treatment is platinum-based chemotherapy. Immunotherapy can be considered a subsequent treatment if the chemotherapy is ineffective or the disease progresses. However, how should treatment be administered if immunotherapy is useless and the patient cannot take chemotherapy? Especially in patients who have metastasized after radical nephroureterectomy (RNU). A 68-year-old woman was admitted to the hospital due to left flank pain for two weeks and was diagnosed with metastatic UTUC after RNU. The patient underwent systemic chemotherapy with gemcitabine and cisplatin (GC), as well as reduced-dose GC, but developed myelosuppression. Immunotherapy was ineffective, so Disitamab vedotin (RC48) was chosen. Disitamab vedotin (RC48) monotherapy was administered for 13 cycles, during which thyroid…
