Louise Burke

TL;DR
This pilot initiative aims to make physical activity more accessible for people with chronic conditions by bridging the gap between healthcare and community programs.

PAFH Officers have engaged with stakeholders and delivered tailored physical activity programs for people with chronic conditions.
Programs like MoveWell and Walking for Chronic Conditions demonstrate practical approaches to inclusivity in physical activity.
The pilot is expected to improve pathways for individuals transitioning from healthcare to community-based activity.
