## TL;DR

This paper reports a rare thyroid cancer called SETTLE and highlights its importance in diagnosis for better treatment and outcomes.

## Contribution

The paper emphasizes the need to consider SETTLE in thyroid cancer differential diagnosis due to its impact on treatment and prognosis.

## Key findings

- SETTLE should be considered in thyroid cancer differential diagnosis, especially in medullary carcinoma cases.
- SETTLE is associated with younger patients and indolent clinical presentation.

## Abstract

Spindle epithelial tumor with thymus‐like element should be included in the differential diagnosis of thyroid gland cancers, particularly in medullary carcinoma, younger patients and indolent clinical presentation, because it may influence treatment and prognosis.
