TL;DR
A patient with poor lung function had thyroid surgery under local anesthesia to avoid risks of general anesthesia before a lung transplant.
Contribution
The paper presents a rare case of thyroid lobectomy under local anesthesia in a high-risk patient due to diffuse panbronchiolitis.
Findings
Local anesthesia was used to avoid risks of general anesthesia in a patient with poor respiratory function.
Techniques like energy devices and frequent local anesthetic injections helped manage pain and bleeding effectively.
Maintaining a slightly upright position and using an anesthesia screen improved surgical access and patient comfort.
Abstract
We herein report a case of thyroid lobectomy performed under local anesthesia for thyroid cancer in a patient who was at a high risk for general anesthesia due to diffuse panbronchiolitis. Although thyroid surgery has been performed in the past under local anesthesia in low-risk patients, thyroid surgery is now rarely performed under local anesthesia. If they are performed, thyroid surgery under local anesthesia is usually performed under monitored anesthesia care; sedation is considered safe and does not cause discomfort to patients. The present patient's respiratory function was poor, raising concerns that once intubated, extubation may not be possible because of the potential deterioration of respiratory function caused by the suppression of spontaneous breathing. Therefore, sedatives were avoided to maintain spontaneous breathing as much as possible. In such high-risk patients,…
